 Ze begon te acteren in een aantal televisie productie. In 1974 begint haar filmcarri?re met de film Thieves Like Us. In deze film werd zijn ontdekt voor een andere film, namelijk One Flew over the Cuckoo?s Nest. Voor deze film kreeg zij zelfs een Oscar voor de beste actrice. Ze is nog steeds bezig met films maken.

Louise Fletcher trouwde in 1960 met de producer Jerry Bick en kreeg van hem twee zonen. Na haar scheiding in 1978 kreeg zij een relatie met James Mason.
